Case Management Services for Persons with Brain Injury

National Rehabilitation and Rediscovery Foundation is a non-profit community based organization dedicated to improving the quality of life for people with brain injuries and other neurological disabilities.

Case managers create a dynamic collaborative process which utilizes and builds on the strengths and resources of the consumer, family members, and community agencies to assist in identifying needs, accessing and coordinating needed services, and achieving short term and long term goals. The case manager's process involves accessing, planning, implementing, coordinating, monitoring and evaluating to promote quality cost effective outcomes.
