Case Management, Intellectual Disabilities

A program of Horizon Behavioral Health, Case Management for people with intellectual disabilities includes these services:
 

• Intake Services is the initial assessment for children and adolescents with intellectual disabilities. Services are available to any child or adolescent, with an intellectual disability, who is in need of targeted case management or waiver services. After intake, referrals are made to the appropriate services by the case manager.
• Case Management Services: Case managers focus on assisting children and adolescents, with intellectual disabilities, and their families with services in their community. These services include but are not limited to: supportive level counseling, linking with community resources, Individualized Education Program (IEP) and school assistance, enhancing community resources, monitoring the quality of services, and accessing needed services.
• Evaluation for Intellectual Disabilities Waiver Services: In order to be eligible for waiver funded services, a child or adolescent must have an intellectual disability, or if under age six, must be at risk for intellectual disability. The child or adolescent will be assessed for their level of functioning and must demonstrate the need for Waiver Services by meeting specific levels of care criteria.
