Who We Are
The long-term community based Case Management Program encompasses identifying individual and family needs and planning for the now and the future. By identifying and coordinating community services to meet the needs and wants of the survivor, monitoring progress, re-evaluating the individual's program and building support teams to assist the individual and their families, case management participants reach their goals.
The case manager supports the individual as he/she plans for the future and moves toward living a life as independently as possible in their community. Goals are established by the individual and are based on their wants and needs; the case manager supports them in person-centered planning and through individualized service plans.
Case management is available for children, youth and adults.
