Supportive Housing Program

Streetlight Community Outreach provides Permanent Supportive Housing for up to  25 individuals who are either disabled, medically fragile or considered "chronically homeless". Each individual receives his or her own bedroom and shares common living areas such as the kitchen and living room.  Supportive services include case management, life skills support, job readiness program, and more.

Supportive services for residential clients include:

• Case Management
• Life Skills Support
• Job Readiness Program
• Assistance with application for mainstream services such as disability benefits & food stamps
• Referral and placement coordination to Recovery Programs
• Transportation to job interviews/doctor visits
