Transitional Care Center

Universal Transitional Care Center is offered by Universal Health Corporation to ease the strain on nursing homes. This service is for patients in transition between an acute stay in a hospital or rehabilitation center and returning to home full-time. This outpatient care for recently discharged patients serves as a bridge for the time when you are well enough to leave the hospital but are not yet strong enough to stop receiving care altogether. Patient needs addressed include:

• easy access to follow-ups
