The consumers in the Community Support Program may have intellectual disabilities and/or developmental disabilities, mental health needs and physical disabilities. The services they receive include support with daily living skills; medication management; monitoring for health/safety; communication development; symptom monitoring and problem-solving; personal care assistance; financial management; linkage for medical/dental/psychiatric needs; community integration/transportation; periodic support hours for illness, vacation and doctor appointments; support with maintaining community jobs; supportive person-centered planning; and in-house activities.
