Who We Are

HopeTree Family Services offers a program of Supported Living/In Home Supports.  The objective is to provide care, assistance and supervision to individuals while they remain in their own home.  These services are supplemental to the care provided by a primary caregiver, including a parent or family member, or they may support an individual whose level of independence does not require a primary care provider.  A plan for each resident is developed, outlining their support needs, as well as specific outcomes expressed by the individual, or evaluated as an area of need/challenge.  The plan is then used to help each individual progressively strengthen his/her own individual knowledge, skills and independence.

Each person supported should be in need of HCBW supports and services. Some homes are not wheelchair accessible.
Other Eligibility Criteria
Individuals with developmental disabilities such as intellectual disabilities or other neurological impairments. Residents may also have physical disabilities.
