Staff provides health care for citizens living in Patrick County, currently without health insurance; and with income at or below 300% of the Federal Poverty level. Staff includes medical doctors, nurse practitioners, physician assistants, registered nurses, nursing assistants, dental hygienists, and women's health nurse practitioners.

Intake Process
Call to make an appointment. Clients must provide the following information at the time of registration: photo ID, demographic information, proof of Patrick County residency, and proof of income for each and all members of the household (SS, Disability, Food Stamps, housing, etc)
