Who We Are

Commonwealth Catholic Charities (CCC) acts as guardian of last resort for incapacitated, indigent adults where no other suitable person is willing or able to serve in such a capacity.  CCC has been designated by the Virginia Department of Aging as a public guardian and conservator provider. Individuals deemed by the court to be incapacitated due to Alzheimer's disease, dementia, brain injury, intellectual disability, or chronic mental illness may need the assistance of a legal guardian. CCC has experienced guardianship specialists to perform the following:

  • Monthly and as-needed visits
  • Care monitoring and communication with other care team  members
  • Assessment, advocacy, and case management
  • Consent for services
  • Annual report to appropriate authorities
  • 24/7 on-call availability
  • Financial management

A guardian makes decisions about where the individual will live, the services they will receive, medical attention, and other aspects of their life, including managing assets and financial matters.
