Appalachian Community Transitions (ACTion), Care Transitions

Appalachian Agency for Senior Citizens (AASC) offers a Free program, Care Transitions, for Medicare beneficiaries.   The program aids in the transition from the hospital to home and aims to educate clients on the risk factors for hospital readmission.



Services cover a 4 week time frame and offer understanding and managing medications, recognizing signs of conditions becoming worse, etc.
