Discharge Express

Care Advantage offers a Discharge Express Program specifically designed to make the patient's transition from hospital to home a seamless, stress-free process.  Services can include: 

• Liaising directly with the hospital contact to ensure the patient is picked up on time
• Transporting patients to the pharmacy or grocery store to pick up prescriptions or supplies
• Helping patients unpack from hospital stays, start laundry, prepare a meal, etc.

Other services available are laundry, bathing, medication reminders, socialization, running errands, transportation, and more.
