Supportive In-Home Services

Community Direct Services provides support and training in the home for people living in their own home and/or with family members. Services are based on the needs of the individual. These services may include training and support necessary for the person to maintain personal care and social skills, complete independent living tasks, medication management, and monitoring of physical and mental health status. Supported home living also helps develop and improve community living skills. Staffing is generally one-to-one unless group activities are specified in the Individual Service Plan (ISP) for therapeutic reasons or personal preference.
